May be required based on staffing needs Key Responsibilities Provide skilled nursing care in patients' homes. Assist with patient assessments, care planning, and documentation. Educate patients and families on self-care techniques. Coordinate care with RNs and interdisciplinary teams. Maintain accurate clinical records and communication notes. Follow infection control and safety protocols. Assist with admissions, transfers, and discharges. Requirements Active LPN/LVN license in the practicing state. Minimum 2 years of Home Health experience. Experience with OASIS assessments. Experience using Homecare Homebase (HCHB). Valid driver's license and reliable transportation. Strong communication and teamwork skills. Education Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing. Certifications Current CPR Certification. Active LPN/LVN License. Preferred Qualifications Home Health or Hospice experience. Knowledge of Medicare documentation standards.
