6:30 AM to 6:30 PM Position Summary The Licensed Nurse in the Memory Care Center provides licensed nursing care and support to residents living with Alzheimer's disease, dementia, and other memory-related conditions. This position is structured as an LPN-level licensed nurse position. An individual holding either an active Kentucky Licensed Practical Nurse license or an active Kentucky Registered Nurse license may be considered for the position; however, the assigned duties, responsibilities, and compensation are based on the requirements of this Memory Care licensed nurse role. The Licensed Nurse works closely with the Memory Care leadership team, direct care staff, residents, families, physicians, and other members of the interdisciplinary team to promote resident safety, dignity, comfort, and quality of life. Essential Duties and Responsibilities Administer medications and treatments as prescribed by physicians or authorized providers, ensuring accurate documentation and compliance with medication administration protocols. Perform routine nursing observations and assessments, including vital signs, resident condition monitoring, and identification of changes in physical, cognitive, behavioral, or emotional status. Notify appropriate leadership, physicians, families, and interdisciplinary team members of significant changes in resident condition, care needs, or safety concerns. Assist with the implementation, review, and updating of resident care plans based on resident needs, physician orders, family input, and interdisciplinary recommendations. Provide direct licensed nursing care, including wound care, catheter care, treatments, monitoring, and support with activities of daily living as appropriate and within the employee's scope of licensure. Support residents with memory-related needs using a compassionate, calm, and person-centered approach. Communicate effectively with residents, families, physicians, pharmacy providers, and interdisciplinary team members to support continuity and coordination of care. Serve as a liaison between residents, families, and healthcare professionals by relaying important information, addressing concerns, and providing appropriate support. Document and maintain accurate, timely, and complete medical records, including nursing notes, medication administration records, treatment records, resident observations, progress notes, and significant events. Monitor residents for signs of distress, decline, infection, behavioral changes, or medical emergencies and respond promptly in accordance with established protocols. Implement and uphold infection control, safety, resident rights, confidentiality, and regulatory compliance standards. Support and provide direction to direct care staff as assigned and within the employee's scope of licensure and delegated authority. Assist with admissions, transfers, discharges, care conferences, audits, and quality improvement activities as assigned. Maintain a safe, secure, respectful, and supportive environment for residents, families, visitors, and staff. Participate in required training, in-services, meetings, and ongoing professional development, including memory care-related education. Perform other duties as assigned. Knowledge, Skills, and Abilities High school diploma or GED required. Active Licensed Practical Nurse license or Registered Nurse license in the state of Kentucky required. This position requires, at minimum, the ability to perform the duties of a Licensed Practical Nurse in the Memory Care setting. Registered Nurses may be considered for this position; however, the position duties and compensation remain assigned to the posted Memory Care licensed nurse role. Knowledge of medication administration, treatment procedures, resident care documentation, infection control, and basic nursing standards. Ability to recognize and report changes in resident condition, behavior, or safety status. Ability to provide compassionate, resident-centered care to individuals with Alzheimer's disease, dementia, and other memory-related conditions. Ability to communicate clearly and professionally with residents, families, staff, physicians, and interdisciplinary team members. Ability to maintain accurate documentation and comply with legal, ethical, regulatory, and facility standards. Ability to work as part of a multidisciplinary team while maintaining professionalism, confidentiality, and accountability. Willingness to attend workshops, seminars, in-services, and specialized training related to memory care, dementia care, long-term care, and geriatric nursing.
