Job Duties:

Triage Coordinate with providers and nurses to resolve health concerns. Focus on assessments. Integrated Care Promote the delivery of care across the continuum of services. Care Gap attainment of identified preventive and chronic disease gaps in care. Failed Appointments Follow-up on cancelled/no show appointments. Lab tracking/Medical

• Monitor lab orders. Data
• Prepare data on Referrals.

Qualifications/Skills:

Current RN or LPN license in good standing with the State of Louisiana (Required). Current Basic Life Support (BLS) (Required). Prefer experience in ambulatory care setting.

Starting Salary Range :

• $18.66
• $24.35 per hour; up to 3% annual increases. RN
• $50,230
• $71,240 per year; up to 3% annual increases.
