In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, assessments, and personalized care plans. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a fast-paced, collaborative environment that values your skills and dedication. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care planning. Administer medications and injections accurately following physician orders and established protocols, including IV infusions and medication management. Assist with diagnostic evaluations such as specimen collection, laboratory testing, and medical imaging procedures. Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or memory care requirements. Manage acute pain and participate in emergency response activities like airway management and ventilator support when necessary. Document all patient interactions thoroughly using EMR (Electronic Medical Records) systems such as Epic or Cerner while maintaining HIPAA compliance. Support discharge planning, case management, and coordination with multidisciplinary teams to ensure continuity of care across settings like hospice, home care, or inpatient units. Skills Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal units, or behavioral health settings. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and CPT coding for accurate documentation and billing. Skilled in performing procedures such as catheterization, suturing, phlebotomy, tube feeding administration, and sterile technique. Proficiency with EMR/EHR systems like Athenahealth or eClinicalWorks for efficient patient documentation. Excellent assessment skills with the ability to recognize signs of infection control issues or acute deterioration requiring immediate intervention. Familiarity with specialized areas such as ICU (Intensive Care Unit), Level I/II trauma centers, surgical units including OR experience or PACU (Post-Anesthesia Care Unit). Ability to work effectively with individuals with disabilities or developmental delays in settings like assisted living or home & community care environments.
