• Live out values of compassion, trust, and excellence every day What You'll Do Provide direct patient care in accordance with the Nurse Practice Act Follow and support care plans developed by the Registered Nurse (RN) Deliver skilled nursing care (treatments, medication support, wound care) Monitor patient conditions and report changes to the care team Educate patients and caregivers on treatments and disease management Maintain infection control and proper use of medical equipment Accurately document care in the EMR system Communicate with physicians and interdisciplinary team members Participate in team meetings, case conferences, and ongoing training What You'll Need Active LPN/LVN license in the state of practice Graduate of an accredited nursing program Current CPR certification 1+ year of nursing experience preferred (home health, med-surg, or acute care) Strong clinical, observational, and communication skills Basic EMR proficiency Valid driver's license and reliable transportation Preferred Qualifications Home Health or community-based care experience Experience with Homecare Homebase (HCHB) Ability to work independently while staying connected to a team What to Expect Field-based role providing care in patient homes Collaboration with RN and interdisciplinary care team Structured but flexible daily schedule Focus on quality care, documentation, and patient experience Benefits Health & Wellness Medical, Dental, Vision Pharmacy benefits Virtual care & mental health support
