QUALIFICATIONS

Licensing/Certification:

Must hold a current, unencumbered license in the State of Louisiana as a Registered Nurse or Licensed Practical Nurse or Licensed Vocational Nurse, as well as possess current certification in CPR. Must be registered with the State Board of Registration in Louisiana.

Education:

Must be a graduate of an accredited educational institution (school of nursing) or a graduate of a state approved school of practical/vocational nursing.

Experience:

At least one year of nursing experience preferred, particularly in wound care.

SPECIAL DEMANDS

: Must be able to work with patients, patients' families, and other nursing personnel to ensure proper care of wounds or other medical conditions.

JOB KNOWLEDGE

: Supervises and sets the standard of care within the facility to assure a professional, skilled program of treatment so that each resident receives the necessary medical, physical, emotional, and psychological support to meet his or her needs. Must operate within the scope of Nurse Practice Act.

STANDARDS

: Adhere to all health and safety standards demonstrating the knowledge and ability to meet STATE, FEDERAL, and OSHA regulations.

DUTIES AND RESPONSIBILITIES

: Perform all wound treatments daily for decubitus ulcers, stasis ulcers, surgical wounds, etc. Document wound appearance, healing, deterioration, etc. at least 3 times a week in nurses' notes. Notify physician and family representative of new wounds or significant change in wound status with accompanying documentation. Notify employee, who orders supplies, what is needed for wound treatments on a routine basis. Complete full body skin assessments on all admits and readmits. Chart in nurses' notes if admitting skin assessment is inaccurate or incomplete. Complete and document weekly skin audits on high-risk residents. Stage all ulcers initially and weekly. Complete QA checklist on all new wounds. Follow-up with physician on any areas that need addressing. Focus on measures to prevent wounds when making rounds or treating a wound (turn schedules posted and followed, moisture barrier used on incontinent residents, heel protectors, heel floated, pressure reducing mattresses, etc.) Provide all Clinical Care Coordinators with a copy of wound report weekly. Alert Clinical Care Coordinators when wounds develop or heal so care plans can be updated. Complete all Non-Pressure Sore Tracking Reports Weekly. Complete Infection Control Weekly Trending. Complete Infection Control Monthly Report. Attend In-service meetings to keep current on wound guidelines and treatment.
