Applicants who do not meet these qualifications will not be considered. Delivering compassionate, skilled care to patients in their own homes, this role allows a dedicated Licensed Practical Nurse (LPN) to have a meaningful impact on geriatric clients through personalized healthcare. Expertise in home health, coupled with proficiency in HomeCare HomeBase (HCHB) EMR, is essential for ensuring high standards of care and accurate documentation throughout the assignment.
Schedule/Hours:
Contract position; duration to be discussed with candidates during the interview process.
Qualifications:
Current, active LPN license Minimum of one year of recent home health clinical experience Proficient in HomeCare HomeBase (HCHB) EMR Strong clinical assessment and precise documentation abilities Reliable transportation for visiting patients' homes Ability to work independently while efficiently managing a caseload
Core Responsibilities:
Delivering skilled nursing care to patients at home, following individualized care plans Monitoring patient progress and documenting updates or changes in condition Administering medications and treatments in alignment with state regulations Educating patients and families on care management, medication adherence, and preventive health practices Maintaining thorough, up-to-date records using
HCHB EMR
