Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs. Administer medications, injections, and other treatments following established protocols while ensuring patient safety and comfort. Maintain accurate medical documentation using EMR (Electronic Medical Records) and EHR . Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, infectious diseases, or those requiring hospice or palliative care. Collaborate with interdisciplinary teams on discharge planning, case management, and patient education to promote recovery and health coaching. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) . Skills in patient assessment, triage procedures, pain management strategies including acute pain management techniques, and infection control practices. Ability to work effectively with individuals with disabilities or developmental challenges in assisted living or home & community care settings. Strong communication skills for patient education and interdisciplinary collaboration; familiarity with medical imaging processes is a plus.
