Monday through Friday, 8-hour shifts
Position Summary:
The Licensed Practical Nurse (LPN) in the Discharge Lobby plays a vital role in ensuring patients leave the hospital with clarity, confidence, and compassion. This position focuses heavily on discharge education, coordination of care, and creating a positive final experience for patients and families. The ideal candidate is detail-oriented, patient-focused, and brings a high level of professionalism and kindness to every interaction.
Key Responsibilities:
Provide clear, thorough discharge education to patients and families, including medications, follow-up appointments, and care instructions Reinforce physician and RN discharge plans to ensure patient understanding and compliance Collaborate with nurses, providers, case management, and support staff to facilitate timely and safe discharges Answer patient questions and address concerns with patience and empathy Ensure all discharge documentation is complete, accurate, and compliant Assist with coordination of transportation and post-discharge needs as appropriate Maintain a welcoming, calm, and organized discharge environment Promote a culture of teamwork, respect, and excellent customer service
Qualifications:
Current Louisiana LPN license in good standing Previous acute care or med-surg experience preferred Strong communication and patient education skills Ability to multitask and manage patient flow efficiently Positive attitude and team-oriented mindset Must demonstrate professionalism, compassion, and the ability to work well with all team members
