In this vital role, you will provide expert medication management and patient care to residents in a supportive, community-focused environment. Your energetic approach and attention to detail will ensure residents receive safe, timely medication administration and personalized care that promotes their well-being and independence. This position offers an exciting opportunity to make a meaningful difference in the lives of seniors and individuals with diverse healthcare needs. Responsibilities Administer medications accurately according to physician orders. Monitor residents' vital signs, assess their condition, and document responses to medication and treatments using electronic health record (EHR) systems . Collaborate with interdisciplinary teams to develop and update individualized care plans tailored to each resident's needs, including dementia care, Alzheimer's care, and memory support. Conduct comprehensive patient assessments, including physical examinations and medication reconciliation, ensuring compliance with HIPAA regulations and medical documentation standards. Provide emergency response when necessary by managing airway issues, acute pain episodes, or other urgent medical situations with skills in airway management, and basic life support. Educate residents and their families on medication regimens, side effects, and health coaching strategies to promote understanding and adherence. Qualifications Valid Licensed Practical Nurse (LPN) license with current state certification. Demonstrated experience working in assisted living facilities or long-term care environments; prior hospital or nursing home experience is highly valued. Proficiency with EMR/EHR systems. Strong understanding of physiology, anatomy, pharmacology, and medical terminology relevant to senior care and dementia management. Experience with medication reconciliation and pain management techniques. Ability to work effectively with individuals with disabilities or developmental challenges; experience in hospice or palliative care is advantageous. Excellent patient assessment skills combined with critical thinking for triage situations or urgent care scenarios.
