Job Description
The Licensed Practical Nurse must be a person that is caring, compassionate, ethical and dependable. In this role, you will be a member of a Community Based Wavier Team. You will be the liaison for the client and allied health team. You will be required to work under the direction of a Registered Nurse. The Licensed Practical Nurse must be flexible to managing daily situations. Licensed Practical Nurse (LPN)
Responsibilities:
In this role, you will be supervised by the Registered Nurse on the Team. As the LPN you work closely with the Residential Director.ESSENTIAL DUTIES AND RESPONSIBILITIES
are as follows: Takes and records clients' vital signs, dresses wound(s), give enemas, douches, alcohol rubs, and massages. Applies hot and/or cold compresses. Observes clients and reports adverse reactions to medication or treatment to medical personnel in charge. Administers specified medication, orally topically, rectally, vaginally, or by subcutaneous or intermuscular injection, and notes time and amount on clients' charts. Collects samples, such as urine, sputum, stool or blood, from clients for testing. Inventories and requisitions supplies. Ensure that lab work and x-rays ordered by physician are done as ordered and reported to the charge nurse/physician. Transcribe physicians' orders and communicate changes to RN and direct care staff before end of shift. Participates in the admission and discharge meetings for residential clients. Communicate with family members regarding the progress of the client in the medical area. Required to perform physical assessments of client under the RN's direction. Participate in the Individual Habilitation Plan (IHP) meetings. Functions as a member of an Inter-Disciplinary treatment team which assesses, plans treatment for and evaluates the progress of each client. Participates in Pharmacy Drug Review and progress review conferences and follow up on recommendations made by consulting pharmacist. Reports medical progress or lack of medical progress to RN. On 24 hour on-call as scheduled by supervisor, requiring a working phone. Visit client homes to assess and monitor clients, monitor accuracy of charting, perform staff training (Required to have reliable transportation). Maintain client rights and confidentiality at all times. Reports any incident of injury immediately to the supervisor on duty and complete written report before the end of the shift. Responsible for knowing and carrying out the established policies and procedures of LARC. Required to be familiar with Safety and Hazard guidelines set by LARC/OSHA and implement such guidelines. Required to attend on-going training. Performs other routine duties as assigned by Registered Nurse.QUALIFICATION REQUIREMENTS

LPNs must complete an approved program and National Council Licensure Examination-Practical Nursing (NCLEX-PN) exam.LANGUAGE SKILLS
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures and governmental regulations. Ability to draft reports, business correspondence and procedure manuals. Ability to effectively present information and respond to questions from Q's, DSW, clients, family, and the general public.REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exist. Ability to interpret a variety of instructions furnished in written, oral, diagram and/or schedule form.CERTIFICATES, LICENSES, REGISTRATIONS
Required criminal history check of personnel working with individuals with disabilities Current State LPN License Nurses Professional Liability Insurance Current CPR and AED Certification OTHER SKILLS andABILITIES
Must have knowledge of clients with intellectual developmental disorders and disorders associated with cognitive function, an ability to use therapeutic techniques. Must be able to take and give instructions well and enjoy working with the disabled.PHYSICAL DEMANDS
