Job Description

Weekend LPN (Licensed Practical Nurse) Braintree, MA 02184 From $32 an hour - Full-time From $32 an hour - Full-time Old Colony Hospice is hiring for an LPN position at the Dr. Ruth McLain Hospice Home in Braintree, MA to work from 7:00am - 7:00pm three days per week. The schedule includes every weekend and Tuesday. The LPN will work 36 hours per week and will be paid for 40 hours per week. The LPN provides physical, emotional, and psychosocial care under the direction of a registered nurse (RN). The nursing care supports a collaborative process that promotes comfort to patients and families experiencing life-limiting illness. The LPN assists the RN by providing the necessary treatments, education and documentation consistent with the plan of care. Observation of patient's basic health status and documentation of findings Collaborate with hospice team in establishing, reviewing, revising the plan of care Assist in providing symptom management for discomfort as specified in the plan of care Assist in performing specialized duties related to end of life care Observe and report patient/family/caregiver response to care Educate patients/caregivers re: disease process, self-care techniques, and end of life process Inform primary RN of changes in patient needs and outcome of interventions Share in providing on-call/continuous care coverage when necessary as supervised by the RN Assist in the orientation of non-licensed individuals to patient needs as identified in the plan of care Documentation must comply with hospice documentation policies and state/federal regulations and accrediting body standards Knowledge and understanding of Hospice Benefit, (Medicare, Medicaid, HMO) Knowledge of state and federal regulations and other accrediting bodies regarding the delivery of nursing services and acceptable standards of practice Responsible for complying with all state, federal, and third party insurance standards; the organization's procedures and protocols, and other industry-wide standards as related to nursing service
Qualifications:
Graduate of an approved practical nursing program and licensed as a Licensed Practical Nurse in the Commonwealth of Massachusetts 2-3 years of applicable experience providing continuous care to patients in end-stage of life preferred but will consider new graduates ; hospice experience preferred Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team Must have knowledge and understanding of applicable federal, state and local hospice, Medicare, Medicaid, HMO regulations, as well as HIPAA and other regulations protecting personal health information of patients Must be able to maintain reliable attendance and be flexible in managing time to respond to needs of the organization with availability for times that are outside of the established schedule for this position Excellent organizational and communication skills Must have proficient computer skills with the ability to learn and apply specialized software Must have reliable transportation and current and valid driver's license and able to travel from patient home, facility and to office as needed on a daily basis Must be able to pass a pre-employment drug test and background check as well as able to provide 3 current professional references that can be checked and verified prior to employment Old Colony Hospice is an equal opportunity employer.
