40 Hours/Week | 8-Hour Mixed Day & Evening Shifts (7:00 AM

  • 11:30 PM) | Every Other Weekend Required Position Overview We are seeking dedicated Licensed Practical Nurses (LPNs) to provide quality nursing care in a Long-Term Care and Skilled Nursing setting.
The LPN will work closely with residents, families, physicians, and interdisciplinary healthcare teams to ensure exceptional resident-centered care while maintaining a safe and therapeutic environment. Responsibilities Provide direct nursing care to residents in a long-term care and skilled nursing environment. Supervise and direct Nursing Assistants during assigned shifts. Administer medications, treatments, and nursing interventions as prescribed. Monitor resident conditions and report changes to physicians and nursing leadership. Complete admissions, transfers, discharges, and required clinical documentation. Maintain accurate resident records and documentation. Participate in care planning and interdisciplinary team meetings. Perform wound care, catheterization, tube feedings, dressing changes, and other nursing procedures within scope of practice. Collect laboratory specimens and perform routine testing as required. Educate residents and families regarding treatment plans and healthcare needs. Ensure adequate supplies, medications, and equipment are available for resident care. Promote resident safety, dignity, comfort, and quality of life. Participate in quality improvement activities and facility initiatives. Required Qualifications Graduate of an accredited Practical Nursing Program. Active Massachusetts LPN License. Current BLS/CPR Certification. AED Certification. Previous Long-Term Care (LTC), Skilled Nursing Facility (SNF), Rehabilitation, or Geriatric Nursing experience preferred. Strong medication administration and clinical assessment skills. Ability to supervise and lead Nursing Assistants effectively. Knowledge of infection control practices and resident care standards. Preferred Qualifications Long-Term Care and Skilled Nursing experience. Wound care and treatment management experience. Leadership and staff supervision experience.
