40 Hours Per Week Position Overview "Navitas Healthcare, LLC" is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality resident care in a Skilled Nursing Facility (SNF) setting. The LPN will support daily nursing operations, administer medications and treatments, supervise support staff, and ensure residents receive exceptional care in accordance with established care plans and facility standards. Key Responsibilities Provide direct nursing care based on individualized care plans and physician orders. Administer medications, treatments, and nursing interventions safely and accurately. Monitor resident conditions and report changes to physicians and nursing leadership. Supervise and support Certified Nursing Assistants (CNAs) and nursing support staff. Complete admission, transfer, and discharge documentation. Receive, transcribe, and implement physician orders. Maintain accurate resident records and clinical documentation. Participate in care planning, assessments, and interdisciplinary team meetings. Perform skilled nursing procedures including wound care, catheter care, tube feedings, specimen collection, suctioning, and dressing changes. Educate residents and families regarding treatments, medications, and care plans. Collaborate with physicians and healthcare team members to support positive resident outcomes. Ensure compliance with regulatory and facility standards. Maintain resident confidentiality and advocate for resident rights. Required Qualifications Education Graduate of an Accredited Practical Nursing Program. Licensure & Certifications Active Massachusetts Licensed Practical Nurse (LPN) License. Current BLS/CPR Certification. AED Certification. Preferred Experience Skilled Nursing Facility (SNF), Long-Term Care (LTC), Rehabilitation, or Geriatric Nursing experience. Medication administration experience in a long-term care setting. Experience supervising CNAs or support staff. Required Skills Strong medication administration and treatment skills. Knowledge of long-term care regulations and nursing standards. Excellent communication and documentation abilities. Strong assessment and critical-thinking skills. Ability to prioritize and manage multiple residents effectively. Compassionate, resident-focused approach to care. Strong organizational and time-management skills.
