This position is responsible for clinical assessments, medication administration, care planning, and ensuring residents receive high-quality, individualized care while maintaining dignity and independence. Key Responsibilities Administer medications and treatments in accordance with physician orders and state regulations Conduct resident assessments and assist in care planning and updates Monitor resident conditions and communicate changes to the care team Ensure accurate and timely documentation of nursing services Support and supervise caregivers and med techs as needed Promote a safe, clean, and compliant care environment Build strong relationships with residents and families Qualifications Active Licensed Practical Nurse (LPN) license in the state of Massachusetts required Previous assisted living, memory care, or long-term care experience preferred Strong knowledge of medication administration and clinical best practices Excellent communication and teamwork skills Ability to work in a fast-paced environment while maintaining attention to detail Compassionate, dependable, and resident-focused approach Physical Requirements Ability to stand, walk, bend, and assist residents for extended periods Ability to lift and support residents as needed in accordance with safety guidelines Exposure to healthcare environments, including communicable diseases Our Core Values Serve with Heart Serve with Purpose Serve with Courage Have Fun While Serving What Our Team Says "It truly feels like a family at Charter."
