LPN In Long Term Care/Rehab Center Pay:
$45.00 - $50.00 per hour depending on Day and Shift Job Summary We are seeking compassionate and dedicated Licensed Practical Nurses (LPN) and Registered Nurses (RN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care in a long-term care/rehab facility. Your expertise in nursing practices, medication administration, patient assessment, and documentation will ensure our patients receive safe, effective, and personalized care. Duties Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations. Assist with complex procedures such as wound suturing, tube feeding, venipuncture, and sterile dressing changes while maintaining aseptic technique. Manage patient documentation using electronic health record (EHR) systems like Epic, Cerner, or Athenahealth to ensure precise medical records and compliance with HIPAA regulations. Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or those requiring memory care and behavioral health support. Support acute pain management strategies and participate in discharge planning to facilitate smooth transitions from hospital or inpatient settings to home or community care. Collaborate with multidisciplinary teams including physicians, case managers, therapists, and social workers to coordinate comprehensive patient care plans. Assist with diagnostic evaluations involving medical imaging or laboratory specimen collection/processing to support accurate diagnoses. Educate patients and their families on health coaching topics such as medication management, nutrition, chronic disease management, and post-acute care instructions. Maintain compliance with all healthcare standards including NCQA standards for quality assurance and infection control protocols. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current certification. Proven experience in diverse clinical settings such as long-term care facilities, hospitals and rehabilitation Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation and patient record management. Skilled in performing advanced nursing procedures including IV insertion/infusion therapy, catheterization, phlebotomy, sterile dressing changes, and airway management techniques such as ventilator support if applicable. . Excellent critical thinking skills for triage decisions and acute care interventions in emergency medicine or urgent care environments. Knowledge of infection control practices aligned with OSHA standards to ensure patient safety at all times. Physical ability to stand for extended periods while providing hands-on patient care; excellent communication skills; compassionate approach; team-oriented mindset.
