LPN:
As a Licensed Practical Nurse with Loyal Home Care Services, you'll be at the center of personalized care plans — delivering top-tier support and forming trusted relationships with clients and families.
Responsibilities:
Deliver skilled nursing care based on the Plan of Care and within scope of MA Nurse Practice Act Support and carry out care plans initiated by the Registered Nurse Provide education and guidance to patients and families on condition management and at-home care Monitor health status, document accurately, and report updates to the clinical team Collaborate with RNs and coordinate additional services (PT, OT, HHA, MSW, etc.) Use medical equipment safely, maintaining infection control standards Stay current on best practices through ongoing clinical development Be an advocate and source of encouragement for your patients What You'll Need to
Join Us:
Active LPN license in Massachusetts Graduate of an accredited LPN or vocational nursing program Minimum of 3 years nursing experience (home health or med/surg preferred) Current CPR/BLS certification Reliable transportation and valid driver's license Up-to-date health certificate & TB test Fluent in English (spoken and written) Demonstrated knowledge of HIPAA & patient confidentiality Must be 18 years or older and authorized to work in the U.S. Perks & Benefits $30 per visit Health, Dental & Vision Insurance 401(k) Retirement Plan Referral Bonus Program Community Liaison (Patient Referral) Program Flexible Scheduling & Autonomy in
