Job Description:

We are seeking a compassionate and skilled LPN to join our home health care team. As a Home Health LPN, you will be responsible for delivering high-quality care to patients in their homes, ensuring they receive the best possible treatment while promoting comfort and recovery. Responsibilities:

Provide direct nursing care to patients in their homes, including assessments, care planning, and nursing interventions. Monitor patient health status and progress, updating care plans as needed. Administer medications, wound care, intravenous (IV) therapy and treatments as prescribed by physicians. Educate patients and families on health conditions, self-care techniques, and treatment plans. Collaborate with interdisciplinary team members to ensure comprehensive care. Maintain accurate and up-to-date patient records in compliance with company policies and healthcare regulations. Promote patient safety, comfort, and overall well-being during visits.

Requirements:

Current and valid LPN license in the state of MA. Minimum of 1 year of nursing experience (Home Health experience preferred). Strong assessment, clinical, and critical thinking skills. Ability to work independently and manage time effectively. Excellent communication and interpersonal skills. Valid driver's license and reliable transportation.
