Job Summary Summary Responsible for providing nursing care to patients in their homes.

Essential Functions Assessing patients' health status and developing individualized care plans. Administering medications and treatments as prescribed by the supervising registered nurse or physician. Monitoring patients' vital signs and reporting any changes to the supervising nurse or physician. Assisting patients with activities of daily living such as bathing, grooming, and dressing. Providing wound care and dressing changes. Performing basic diagnostic tests such as blood pressure checks and glucose monitoring. Educating patients and their families on their medical conditions and self-care techniques.
