Temporary Join Our Team as a PACT Nurse (RN or LPN) Are you a compassionate Registered Nurse or Licensed Practical Nurse passionate about helping individuals with serious mental health challenges achieve recovery and independence? We are seeking a dedicated PACT Nurse (RN or LPN) to join our multidisciplinary Assertive Community Treatment (PACT) team in Quincy, MA. What You'll Do As a PACT Nurse, you'll play a vital role in delivering comprehensive, community-based behavioral health services by: Conducting psychiatric and physical health assessments. Developing, implementing, and monitoring individualized treatment plans. Administering and monitoring medications, including evaluating effectiveness and side effects. Providing case management, supportive counseling, and symptom management. Collaborating with psychiatrists, medical providers, families, and community partners. Supporting individuals in achieving housing, employment, wellness, and recovery goals. Responding to behavioral health crises and providing intervention as needed. Participating in team-based treatment planning and care coordination. Maintaining accurate clinical documentation and progress notes. Qualifications Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts. Minimum of 2 years of experience in behavioral health, psychiatric nursing, or community mental health. Strong assessment, communication, and crisis intervention skills. Experience working with individuals experiencing serious mental illness and co-occurring substance use disorders preferred.
