Day Shift | 3 Days per Week | 12-Hour Shifts | 36 Hours per
Week Position Overview:
Navitas Healthcare, LLC is seeking Licensed Practical Nurse (LPN)
  • Long Term Care (LTC) for an exciting Travel or Local job in Franconia, NH.
Responsibilities:
Provide direct patient care in accordance with resident care plans and facility policies Assist in implementing and evaluating individualized resident care under RN supervision Administer medications and treatments as permitted by state regulations and facility policies Monitor resident conditions and promptly report changes to RNs or physicians Collaborate with interdisciplinary teams to support resident outcomes Assist with patient rounds and address resident and family concerns Maintain accurate and compliant documentation of care provided Participate in quality improvement initiatives and audits Provide guidance and support to CNAs and junior staff under RN supervision
Requirements:
Completion of an accredited Practical Nursing program required Successful passage of the
NCLEX-PN
examination required Active Licensed Practical Nurse (LPN) License required Basic Life Support (BLS) Certification required
COVID-19
Vaccination Card required Strong clinical skills and patient care knowledge in long-term care settings Knowledge of long-term care regulations, policies, and compliance standards Effective communication, teamwork, and organizational skills
Preferred Qualifications:
ACLS Certification preferred IV Certification preferred 0-2 years of practical nursing experience preferred Skilled Nursing Facility (SNF) or Long-Term Care experience strongly preferred
