RESIDENT CARE DIRECTOR

The John Bertram Homes of Swampscott and Salem are looking for an experienced nurse manager to run our Wellness Department. The Resident Care Director is responsible for managing the Wellness Nurses and the SAMM medication management program. The Resident Care Director also works in close partnership with our Assisted Living Coordinators on all aspects of resident care.

RESPONSIBILITIES

• Be a part of an evolving continuum of care through the use of a holistic approach to the health and wellness of a geriatric population.
• Serve as a clinical leader and collaborate with other leaders to meet the comprehensive health care needs of geriatric residents in a community setting.
• Collaborate with our Assisted Living Coordinators to perform and document resident assessments and complete Individualized Service Plans.
• Coordinate with physicians, other professional partners, and Bertram House team members to champion an integrated approach to resident care and promote positive clinical outcomes.
• Hire, train, coach, and schedule the nursing team, and the medication managers.
• Lead the community medication management programs through the application of principles of safety in the medication use process.
• Lead clinical quality and process-improvement efforts through the tracking, trending and evaluating of clinical data to recognize clinical excellence and identify areas of opportunity.
• Responsible for management of Wellness Department budget.
• The Resident Care Director is responsible for ensuring regulatory compliance and, in partnership with the rest of the management team, our quality-of-care program.

QUALIFICATIONS

• At least five years of experience as nurse in an assisted living facility.
• At least two years of nursing management experience to include hiring, coaching, performance management, scheduling, and daily operations supervision.
• Experience tracking, trending, and analyzing clinical performance data preferred.
• Strong leadership and supervisory ability, excellent communication skills The position may be filled by an LPN or RN.

Compensation will be commensurate with licensure, experience, and qualifications.
