Job Description

Job # 1108- Saturday & Sunday 8:00am-4:00pm. 16 hrs/wk General Description LPN works directly with the ABI Coordinator, Healthcare Supervisor and RN Manager to provide appropriate medical care to residents within the ABI Residential setting. The LPN participates in relevant training opportunities, oversees the health and wellness of individuals in concert with the RN Manager and Healthcare Supervisor. The LPN reports weekly, and as needed, to the RN Manager. Minimum Qualifications LPN licensure in the state of MA Minimum of two years equivalent work experience with individuals with DD/ID and/or ABI Knowledge of community system serving adults with ABI Comprehensive knowledge of nursing care A safe driving record, current driver's license, personal vehicle, and current motor vehicle liability insurance required Job Responsibilities This position reports directly to and receives support and supervision from the Director of Community Living and/or ABI Program Manager Administer all medication(s) and medical treatment(s) needed during the hours of support Provide as appropriate the services of a liaison to doctors, hospitals, and other support services Must be able to work a flexible schedule with some evening shifts and one weekend day required. Monitor supported individual's health status Communicate effectively and professionally with all team member's program including supported individual, family, and supervisor Implement goals as stated in the program Treat individual/family with dignity and respect Support individual in increasing or maintaining independence within their homes or communities Encourage human rights Support individual in achieving and maintaining personal well-being and safety Provide transportation as needed Respect the individual/family's cultural norms Complete required documentation and reporting. Maintain required certifications (LPN licensure, CPR/AED for Healthcare Providers, Services/Supports, etc.).
