Position Overview:

As a Medication Management LPN (Licensed Practical Nurse) , you will play a crucial role in ensuring the safe and effective administration of medications for our clients. You will conduct in-home visits, educate patients and caregivers, and collaborate with healthcare professionals to enhance patient well-being. Key Responsibilities of LPN (Licensed Practical Nurse) : Conduct in-home assessments to review and manage client medications. Educate clients and caregivers on proper medication usage , potential side effects, and adherence. Set up and organize medications, including pre-filling medication planners when necessary. Collaborate with physicians, pharmacists, and the care team to address medication-related concerns. Monitor clients for adverse reactions and report any significant health changes. Maintain accurate documentation of medication administration and client progress. Ensure compliance with healthcare regulations and company policies. Conduct thorough health assessments , including checking vital signs and implementing care plans.

Qualifications:

Active LPN (Licensed Practical Nurse) license in the state. Experience in medication management, home health, or geriatric care preferred. Strong attention to detail and excellent communication skills. Ability to work independently and provide compassionate, patient-centered care. Reliable transportation for home visits.
