We are seeking Part Time LPNs to join our team! This position presents the opportunity to transition to a Full Time position. The nurse we are looking for will report to our Worcester office and will perform visits in Worcester and surrounding towns.

Responsibilities:

Administer nursing care to ill, injured, or disabled patients Administer patient treatment according to established plan of care. Monitor and report changes in patient symptoms or behavior Communicate with collaborating Case Managers, physicians or specialists regarding patient care Educate patients about health maintenance and disease prevention Facilitate referrals to other healthcare professionals and medical facilities Maintain accurate patient medical records Provide advice and emotional support to patients and their family members

Qualification:

LPN license in good standing in MA. Previous experience in nursing Home care experience is highly desired.
