This position requires flexibility to float between multiple units while delivering compassionate, patient-centered care to adult and geriatric populations.

36 Hours Requirements Active Maryland LPN License or Compact License. Current BLS Certification. Minimum 1-2 years of LPN experience required. Previous LTAC, Skilled Nursing Facility (SNF), Rehabilitation, Long-Term Care, or Pulmonary Care experience preferred. Cerner EMR experience required. Strong clinical assessment and communication skills. Ability to care for patients with complex medical, behavioral health, respiratory, and rehabilitative needs. Experience with dementia, memory care, tracheostomy care, wound care, and chronic disease management preferred. Responsibilities Provide direct nursing care to adult and geriatric patients in LTAC, rehabilitation, pulmonary, memory care, and long-term care settings. Administer medications and treatments according to physician orders and facility protocols. Monitor and document patient conditions, vital signs, intake and output, and response to treatment. Communicate significant changes in patient status to the RN and healthcare team. Perform wound care, dressing changes, blood glucose monitoring, specimen collection, and catheter care. Assist with feeding tube management, tracheostomy care, respiratory support, and oxygen therapy as needed. Support patients with behavioral health conditions, dementia, cognitive impairments, and complex medical needs. Implement infection control practices and maintain compliance with safety regulations. Collaborate with physicians, therapists, respiratory therapists, pharmacists, social workers, and interdisciplinary care teams. Assist with admissions, transfers, discharges, and patient education. Maintain accurate and timely documentation in the electronic medical record system.
