Job Title:
Private Duty Nurse (PDN) -
LPN or RN Job Summary:
Our well-established home health agency is currently seeking an exceptional Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a Private Duty Nurse (PDN). The successful candidate will provide skilled nursing care to a medically complex patient in their home setting, focusing on tracheostomy care, gastrostomy tube (G-tube) management, ventilator support, and other essential private duty nursing responsibilities. This position requires a high level of expertise, compassion, and dedication to patient-centered care.
Key Responsibilities:
Perform comprehensive nursing assessments, develop individualized care plans, and implement nursing interventions to meet the patient's unique needs. Provide expert tracheostomy care, including suctioning, stoma care, and tracheostomy tube changes as needed. Manage and maintain the patient's G-tube, including formula preparation, feeding, medication administration, and stoma care. Operate, troubleshoot, and maintain ventilator equipment, ensuring proper settings and patient response. Administer medications via various routes (oral, enteral, parenteral) and monitor for therapeutic effectiveness and potential side effects. Monitor and record the patient's vital signs, respiratory status, neurological function, and other relevant clinical data. Collaborate with the patient's multidisciplinary healthcare team, including physicians, specialists, therapists, and social workers, to ensure seamless care coordination. Educate and train family members and caregivers on proper care techniques, equipment use, and emergency response protocols. Maintain meticulous documentation of all nursing interventions, assessments, and patient outcomes in accordance with agency policies and regulatory requirements. Participate in quality improvement initiatives, case conferences, and professional development opportunities to enhance patient care and nursing practice.
Qualifications:
Current, unencumbered LPN or RN license in the state of practice. Minimum of 3 years of clinical experience, with at least 2 years in a home health, private duty, or critical care setting. Demonstrated expertise in tracheostomy care, G-tube management, ventilator support, and other complex nursing interventions. Strong critical thinking, problem-solving, and decision-making skills. Excellent verbal and written communication skills, with the ability to effectively interact with patients, families, and healthcare professionals. Proficient in using electronic medical records and documentation systems. Current certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS). Ability to work independently, prioritize tasks, and manage multiple responsibilities in a dynamic home care environment. Flexibility to work various shifts, including nights, weekends, and holidays, to ensure continuity of care.
