In this vital role, you will provide compassionate, comprehensive nursing care in a dynamic environment that emphasizes patient-centered approaches. Your skills will ensure safe, effective treatment and support for individuals with diverse medical needs, including those with chronic conditions, memory impairments, and developmental disabilities. This position offers a rewarding opportunity to make a meaningful difference every day while working within a collaborative healthcare team committed to excellence. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems. Administer medications via injections, IV infusions, and other routes while adhering to HIPAA regulations and sterile technique standards. Manage complex care plans involving dementia care, Alzheimer's care, memory care, and behavioral health considerations. Provide specialized nursing interventions such as tube feeding management, catheter care, wound dressing changes, and acute pain management. Assist with diagnostic evaluations including specimen collection/processing, medical imaging coordination, and basic laboratory procedures. Collaborate with multidisciplinary teams on discharge planning, case management, and patient education to promote optimal health outcomes. Ensure infection control protocols are strictly followed and maintain accurate medical documentation for all patient interactions. Support patients with emergency or urgent care needs by triaging symptoms and coordinating appropriate interventions within the adult day care setting. Experience Valid LPN license with current state licensure required; additional certifications such as Basic Life Support (BLS) preferred. Demonstrated experience in adult outpatient settings such as medical offices, clinics, or community health programs. Prior work in long-term care facilities including assisted living or nursing homes is highly desirable. Experience with complex patient populations including those with dementia, developmental disabilities, or chronic illnesses like infectious diseases or cardiac conditions. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks to document patient care accurately. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and medical terminology relevant to adult healthcare. Exposure to acute care environments such as hospital medicine units or emergency rooms is advantageous but not mandatory. Ability to work independently while collaborating effectively within a multidisciplinary team dedicated to compassionate senior and adult patient care.
