Glance Home Care:

Assist with ADLs, meal prep, hydration, and setup of medical equipment (e.g., commode). Organize medications into pill boxes and create Medication Administration Record (MAR). Educate patient on medication schedule and purpose. Set up a medication calendar and coordinate follow-up doctor appointments. Fill out appointment and medication logs; contact home health for care needs assessment. Explore potential eligibility for VA/respite care.

Hospital:

Ensure patient discharge readiness, including removal of personal items and reconciliation of medication records. Consult with care team about obtaining new or refilled medications (onsite or patient's preferred pharmacy). Transport patient home (stop at pharmacy if needed).

Requirements:

LPN State License BLS Certification Annual Physical Exam TB Test or Chest X-ray Yearly FLU vaccine Fully

COVID-19

Vaccinated Minimum one year of experience required
