12-hour Day & Overnight shifts About the Role We are seeking a skilled and clinically confident Licensed Practical Nurse (LPN) to join a structured skilled nursing facility (SNF) and long-term care (LTC) environment. This role is best suited for nurses who are comfortable with medication administration, clinical assessment, EMR documentation, and managing complex geriatric care needs in a team-based setting. We value consistency, clinical judgment, and commitment to high-quality, resident-centered care. Key Responsibilities Provide direct nursing care in a skilled nursing / long-term care setting Administer medications and treatments with accuracy and clinical judgment Assess residents and promptly communicate changes in condition Maintain accurate and timely EMR documentation Collaborate with RNs, CNAs, providers, and interdisciplinary teams Support individualized care plans and resident goals Ensure compliance with Maine Nurse Practice Act and regulatory standards Maintain a safe, respectful, and structured care environment Requirements Active Maine LPN license (required) Graduation from an accredited practical nursing program Minimum 1 year experience preferred in SNF, LTC, or acute care Strong medication administration and clinical assessment skills Solid EMR documentation habits and attention to detail Ability to work effectively in a structured, team-based clinical environment Strong communication and professional accountability New graduates may be considered if they demonstrate strong clinical readiness and commitment to long-term care.
