  • Part-time, Travel nursing, Contract Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team.

  • 15 weeks Responsibilities Deliver comprehensive nursing care to residents in accordance with physician orders, facility policies, and state regulations.
Conduct patient assessments, monitor vital signs, and document medical information accurately using electronic health record (EHR) systems like Epic or Cerner. Administer medications, injections, IV infusions, and treatments such as tube feedings and wound care following sterile techniques and safety protocols. Assist with activities of daily living (ADLs), including mobility support, catheter care, and basic hygiene to enhance resident comfort. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care services. Respond promptly to urgent situations such as acute pain episodes or emergencies by utilizing airway management skills and emergency protocols. Maintain compliance with HIPAA regulations and ensure meticulous medical documentation for legal and quality assurance purposes. Qualifications Valid license as a Licensed Practical Nurse (LPN) in the state of employment. Experience working in long-term care facilities, nursing homes, assisted living or similar settings preferred. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with medical documentation is highly desirable. Knowledge of geriatrics, dementia care—including Alzheimer's and memory care—and chronic disease management such as diabetes or infectious diseases. Familiarity with IV infusion techniques, medication administration including injections and venipuncture, and infection control practices. Ability to perform physical examinations, vital signs assessment, and patient evaluations confidently. Strong communication skills to effectively triage patient needs and collaborate with healthcare teams; experience in emergency medicine or ICU settings is a plus. Working knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and familiarity with medical terminology is advantageous. Join us in delivering exceptional care that enriches lives!
