Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) specializing in long-term care and travel assignments! In this vital role, you will provide compassionate, high-quality nursing care to residents in long-term care facilities, ensuring their comfort, safety, and well-being. Your expertise will support diverse patient needs, from memory care and dementia to acute pain management and post-acute recovery. This position offers an exciting opportunity to travel between facilities, bring your clinical skills to various settings, and make a meaningful difference in patients' lives every day. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs. Administer medications via injection, IV infusion, and other routes while adhering to HIPAA regulations and medical documentation standards. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques. Manage airway and ventilator support for patients requiring respiratory assistance; monitor for signs of infection or acute distress. Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner; ensure compliance with
ICD-9/10
coding standards. Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes. Provide specialized care including Alzheimer's/dementia/memory care, hospice support, pediatric or neonatal care when needed; work with individuals with disabilities or developmental challenges. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience in long-term care facilities such as nursing homes or assisted living communities. Critical care experience including ICU or emergency room settings highly preferred; familiarity with Level I/II trauma centers is advantageous. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and diagnostic evaluation procedures. Skilled in the use of EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks; proficient in medical documentation and medical records management. Experience with IV insertion/injection administration, phlebotomy, specimen collection/processing, and infection control protocols. Ability to work independently while collaborating effectively within multidisciplinary teams; excellent patient assessment skills are essential. Physical ability to stand for extended periods, lift patients safely using Hoyer lifts or similar equipment, and perform necessary clinical tasks efficiently.
