Job Description

Overview We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team, providing compassionate and skilled patient care across various settings. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency with electronic health record (EHR) systems. This role offers the opportunity to work in a dynamic environment, supporting patients in need of acute, chronic, and long-term care. Candidates with experience in critical care, geriatrics, pediatrics, or specialized areas such as hospice or infectious disease are highly encouraged to apply. The position is paid and open to licensed practical nurses committed to delivering high-quality care while adhering to HIPAA and infection control standards. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and history taking. Administer medications and injections accurately following physician orders and protocols. Perform IV insertions, catheterizations, phlebotomy, and other invasive procedures with sterile technique. Monitor patients' conditions using advanced skills such as ventilator management, airway management, and pain control techniques. Document all patient interactions thoroughly in EMR/EHR systems like Epic, Cerner, or Athenahealth while maintaining compliance with HIPAA regulations. Assist with diagnostic evaluations including laboratory specimen collection/processing and medical imaging procedures. Provide wound care, suture removal, dressing changes, and other post-procedure treatments. Support discharge planning and coordinate care across inpatient and outpatient settings including home & community care. Educate patients and families on health management topics such as medication administration, nutrition, dementia care (Alzheimer's & memory care), and chronic disease management. Collaborate with multidisciplinary teams in settings such as ICU, emergency room, neonatal ICU (NICU), pediatric units, or hospice care to ensure optimal patient outcomes. Maintain infection control standards and aseptic techniques during all procedures. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth. Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation processes. Experience working in diverse healthcare environments including hospital medicine (ICU/Level I & II Trauma Centers), nursing homes, assisted living facilities, or post-acute care settings. Skills in IV infusion therapy, medication administration (including injections), tube feeding management, and ventilator support preferred. Ability to perform physical assessments and vital signs accurately across different patient populations including pediatrics (neonatal ventilator) to geriatrics (dementia & Alzheimer's care). Knowledge of infection control practices and sterile processing techniques. Excellent communication skills for working with patients with disabilities or developmental delays as well as family members. Ability to work independently while collaborating effectively within a healthcare team.
