Wound Care Nurse- LPN Optalis Health and Rehabilitation - 2.5 Bloomfield Hills, MI Job Details 23 hours ago Qualifications Patient assessment LPN Full Job Description Position Overview We are seeking a dedicated and experienced Wound Nurse Coordinator to join our healthcare team. The ideal candidate will be responsible for coordinating wound care services, providing expert nursing care to patients with acute and chronic wounds, and collaborating with multidisciplinary teams to ensure optimal patient outcomes. Key Responsibilities Assess, plan, implement, and evaluate wound care treatment plans for patients. Coordinate wound care services across departments and with external providers as needed. Provide direct nursing care and education to patients and their families regarding wound management and prevention. Monitor patient progress and adjust care plans accordingly. Maintain accurate and detailed documentation of wound assessments, treatments, and patient outcomes. Serve as a resource and mentor to nursing staff on wound care best practices and protocols. Collaborate with physicians, therapists, and other healthcare professionals to develop comprehensive care plans. Stay current with advances in wound care techniques, products, and research. Ensure compliance with regulatory standards and facility policies related to wound care. Qualifications Licensed Practical Nurse (LPN) license in the state. Certification in wound care (e.g., Wound Care Certified (WCC), Certified Wound Specialist (CWS), or equivalent) preferred. Minimum of 3 years nursing experience, with at least 1 year in wound care or related specialty. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Proficient in electronic medical records (EMR) and documentation. #
