Resident Care Coordinator - RN / LPN

Pay:

$28.00-32.00 per hour Job description: Resident Care Coordinator Green Acres of Cadillac Green Acres of Cadillac is seeking a Resident Care Coordinator to join our team. This role plays a vital part in supporting residents, staff, and families while maintaining high-quality care standards within our community.

Key Responsibilities:

Conduct and oversee level of care assessments Coordinate and monitor resident care needs Work closely with physicians, hospice providers, and families Support, educate, and collaborate with direct care staff Order and track medications/scripts as needed Complete medication cart audits Enter and manage resident care orders Communicate effectively with residents, families, and leadership Assist with maintaining compliance with care standards, policies, and regulations Share on-call responsibilities as part of the leadership team

Qualifications:
