Per Diem LPN Schedule :

No scheduling mandation Work an average of three (3) shifts per calendar month. Regular weekend coverage is expected. Must be available to work a minimum of two (2) holidays per year.

This Role:

• Administer medications, perform treatments, monitor vital signs and wound care. Conduct Health Assessments
• Evaluate residents and report changes in condition to the clinical team. Monitor and Adjust Care Plans
• Observe residents for adverse reactions or side effects and update care plans accordingly. Coordinate Interdisciplinary Care
• Work with teams to enhance residents' quality of life. Supervise and Mentor Staff
• Oversee nursing assistants and support staff, providing guidance and mentorship. Document Resident Care
• Complete daily and monthly records accurately, reflecting care provided and resident responses. Manage Admissions and Discharges
• Perform admission and discharge procedures as directed. Maintain Medical Records
• Keep accurate, up-to-date records in compliance with policies and regulations. Ensure Safety and Infection Control
• Follow all protocols to maintain a safe environment for residents and staff. Prevent Spread of Illness
• Adhere to proper infection control procedures at all times. Engage in Ongoing Education
• Maintain certifications and stay current with best practices.

If selected for the position you will receive a full job description. Skills, Knowledge & Expertise Graduate of an accredited School of Nursing (LPN). Active State of Michigan LPN License. CPR certification required or willing to obtain upon hire. Minimum of one (1) year of experience working with the frail and elderly. Ability to safely lift, push, pull, and carry up to a minimum of 50 lbs. Tasks involving more than 50 lbs. require assistance or assistive equipment for safe performance.
