Job Description

Working/Functional Title Triage LPN Position Summary Job Summary:
The Licensed Practical Nurse (LPN) provides direct patient care and clinical support within the Surgical and Specialty Services Clinic under the supervision of a registered nurse or provider. Responsibilities include assisting with patient assessments, administering medications and treatments, documenting clinical information, coordinating care, and supporting a positive patient experience while ensuring high standards of quality and safety.
Key Responsibilities:
Provide direct patient care under the supervision of a registered nurse or provider Obtain and document patient histories, vital signs, and clinical information Administer medications, injections, and treatments as ordered Assist providers with examinations, procedures, and patient care activities Monitor patient conditions and report changes to the healthcare team Educate patients and families on treatment plans, medications, and follow-up care Maintain accurate and timely electronic medical records documentation Coordinate patient scheduling, referrals, and follow-up appointments Ensure compliance with infection control, safety standards, and clinic policies Support a positive, patient-centered environment through compassionate care and teamwork Other duties as assigned Minimum Requirements Knowledge normally acquired from specialized training such as that acquired in one year of college, technical, vocational, or trade school in a Licensed Practical Nurse program with relevant course work in Pharmacology; one to three years of related and progressively more responsible or expansive work experience in a hospital or ambulatory care clinic; requires computer experience or an equivalent combination of education and experience; and registration as a Licensed Practical Nurse. Triage experience required.
