Licensed Practical Nurse (LPN) - Full Time | Home Health Northern Michigan (50 mile radius from Gaylord) | Full-Time | Supportive Team | Company Vehicle Provided Are you a compassionate LPN looking for a rewarding career in home health care? Join our growing, dedicated, locally owned agency and make a real difference in the lives of patients while enjoying flexible scheduling and strong clinical support. We're currently hiring a Full-Time Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the comfort of their homes. Company vehicle and fuel card provided. This position comes with a guaranteed minimum salary with a per visit productivity bonus , ensuring stable income while providing meaningful, patient-centered care.
What You'll Do:
Deliver direct nursing care under the supervision of an RN Case Manager Monitor patient conditions and report changes to the care team Administer medications and treatments as prescribed Educate patients and caregivers on care routines and health management Document all care provided in our user-friendly EMR Collaborate with an interdisciplinary team to coordinate high-quality care

Growth and Continued Education Qualifications:
Active LPN license in the state of Michigan 1+ year of nursing experience (home health preferred but not required) Strong communication and documentation skills Compassionate, reliable, and self-directed Valid driver's license and reliable transportation
