What You'll Do Facilitate the integration of new participants into the health care delivery system including meds, immunizations, routine monitoring of chronic problems, and foot & wound care Administer and monitor participant meds prescribed by the primary care providers and reconcile end of month MAR Sign for medications delivered by the pharmacy daily; request refills of meds in a timely manner.
