More about our team PortagePointe is a long-term care facility with 60 beds to offer the senior community and is known as the premier nursing home serving the entire Upper Peninsula. We are a certified Eden Alternative facility and take great pride in providing a home-like environment to residents. PortagePointe is regulated by the Centers for Medicare and Medicaid (CMS), a member of the American Health Care Association (AHCA), and a member of the Healthcare Association of Michigan (HCAM). How you'll contribute A Post Acute LPN who excels in this role: Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment. Documents patient care given. Monitors, records, and communicate patient condition as appropriate to care team, physician, patient, and family. Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Works as an advocate for the physical and emotional well-being of the patient. Nurtures, sustains, and protects each Elder by providing the services needed to live a full life. Incorporates the Eden Philosophy of living into every action undertaken throughout one's employment. Encourages each Elder to reach and maintain his/her maximum functional ability. Creates opportunities for the Elder to give care to others. Committed to the development of the staff, Elders, and families as collaborative team members.
