Available for full-time employees Rosewood Healthcare is looking for an experienced Licensed Practical Nurse (LPN) to join our team in Mount Pleasant, Michigan. We are looking for someone who is caring, organized, reliable, and enjoys helping patients feel supported. This position is ideal for someone who has experience in a medical office and is comfortable communicating with patients, providers, pharmacies, labs, and referral offices. About the Role The Patient Care Coordinator helps guide patients through their care. You will be an important part of making sure patients understand the next steps, follow through with appointments, complete testing, and receive the support they need. Responsibilities Contact patients by phone or message regarding appointments, results, referrals, and follow-up care Help patients understand and complete the next steps in their care plan Coordinate referrals, labs, imaging, prior authorizations, and specialist appointments Communicate with providers, pharmacies, insurance companies, and other healthcare offices Assist with patient rooming, vitals, medication updates, and clinical documentation as needed Help keep patient care organized and on schedule Provide kind, professional, and respectful communication to every patient Support the medical team with daily office flow and patient care needs Ideal Candidate We are looking for someone who is: An experienced LPN Reliable, professional, and compassionate Comfortable talking with patients on the phone Organized and able to follow through on tasks Good with documentation and attention to detail Able to work well with providers and team members Willing to learn, grow, and be coached Passionate about helping patients navigate the healthcare system Requirements LPN license preferred Prior experience in a medical office strongly preferred Strong communication and computer skills Ability to multitask in a busy healthcare setting Positive attitude and strong work ethic Schedule Full-time position in Mount Pleasant, Michigan. Compensation Starting pay is $18/hour , with benefits available for full-time employees.
