Position Type:
Part-time | Hourly Wellspring Lutheran Services is a statewide nonprofit dedicated to serving seniors, families and children throughout Michigan. We are currently seeking a Wound Care Nurse (RN / LPN) to join our Covenant Skilled Nursing and Rehab at Wellspring team in Saginaw. Covenant Skilled Nursing and Rehab at Wellspring is a partnership between Covenant HealthCare and Wellspring to help patients transition from the hospital to home. Responsibilities Follow all established skin management protocols. Apply current standards of practice for treatment of pressure ulcers, non-pressure ulcers, skin tears and other skin impairments. Stage and measure wounds and document per protocol, including skin conditions other than wounds. Assess all new/readmissions for potential skin or wound concerns within 24 hours in conjunction with the admitting nurse. Serve as a resource for staff and provide ongoing education on skin and wound care. Complete required QA reports and report issues to the DRC. Ensure all preventative measures are implemented according to the resident's or client's plan of care. Provide information for MDS and participate in care conferences as requested. Provide individualized attention to support each resident's independence and overall well-being. Make weekly rounds with the wound care physician and function as part of the IDT in coordinating care. Receive, implement and document new orders regarding skin and wound care from the physician and update the care plan accordingly. Review resident care plans for appropriate goals, problem approaches and needed revisions. Delegate responsibilities for direct wound and skin care as appropriate. Qualifications Licensed by the State of Michigan Board of Nursing as a Registered Nurse (RN) or Licensed Practical Nurse (LPN). Successful completion of an accredited Registered Nursing or Licensed Practical Nursing program. Two years of nursing experience preferred, with current home health, medical, surgical or critical care experience. Wound care experience preferred. Strong communication, critical thinking and organizational skills. Commitment to providing compassionate, high-quality care in a team environment.

3 days / week (Monday, Wednesday & Friday)
Shift:
8:00am

Weekends:
None On-call:
Rotating schedule with nursing management Starting pay: $32.00 /hour (LPN)
  • $37.
25 /hour (RN), based on experience.

$2,500 (LPN)
