Job Description

Job Summary We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including long-term care, assisted living, hospitals, and outpatient clinics. This role involves performing assessments, administering treatments, and collaborating with multidisciplinary teams to ensure optimal patient outcomes. The LPN will utilize their expertise in medical documentation, medication administration, and patient assessment to support our mission of delivering exceptional care to diverse patient populations, including seniors, individuals with developmental disabilities, and those requiring specialized treatments such as IV infusions and hospice care. Duties Conduct comprehensive patient assessments including vital signs, physical examinations, and health histories using EMR/EHR systems such as Epic, Cerner, and Athenahealth. Administer medications and injections accurately following HIPAA guidelines and medical protocols. Provide specialized care including IV infusion, catheterization, tube feeding, ventilator management, and wound suturing. Assist with diagnostic evaluations such as laboratory specimen collection/processing, medical imaging procedures, and cardiac catheterization. Manage airway and respiratory support including airway management techniques and neonatal ventilator care. Document all patient interactions meticulously in medical records ensuring compliance with
ICD-9/ICD-10
coding standards. Support discharge planning, case management, and post-acute care coordination for diverse patient needs. Deliver age-specific care in pediatrics, neonatal units (including neonatal ventilator), geriatrics (including Alzheimer's and dementia care), and behavioral health settings. Collaborate with physicians during emergency room procedures, ICU treatments (including Level I & II Trauma Center experiences), and surgical interventions such as OR procedures. Educate patients on health coaching topics like nutrition, medication management, family planning, and chronic disease management. Maintain infection control standards through aseptic techniques and infection prevention protocols. Qualifications Valid Licensed Practical Nurse (LPN) license in the state of practice. Proven experience in various healthcare environments such as hospitals (including ICU & trauma centers), nursing homes, outpatient clinics, or home & community care settings. Proficiency with EMR/EHR systems including Epic, Cerner, eClinicalWorks; familiarity with
ICD-9/ICD-10
coding is preferred. Strong knowledge of physiology, anatomy, pharmacology related to medication administration and IV infusion therapies. Experience with specialized procedures such as phlebotomy, venipuncture, sterile processing techniques, dialysis support, and dermal fillers is advantageous. Excellent patient assessment skills with the ability to work with diverse populations including children (pediatrics & neonatal), seniors (geriatrics & memory care), and individuals with disabilities or developmental challenges. Ability to work independently while collaborating effectively within multidisciplinary teams in fast-paced environments such as emergency rooms or urgent care settings. Knowledge of HIPAA regulations ensuring confidentiality of patient information.
