Job Description

Job Summary:
Center for Pain Management is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) with experience or interest in pain management to join our team. The LPN will assist our clinic provider and serve as a liaison between the provider and patients in order to provide outstanding patient care and support.
Key Responsibilities:
Clinical Support:
Rooming patients Assist provider with clinic-based procedures Review patient charts and medication history, particularly related to chronic pain conditions and controlled substances. Provide patient education Assist with prior authorizations, medication refills, and documentation of narcotic agreements and compliance.
Telephone Triage:
Answer and return patient phone calls promptly and professionally. Assess patient symptoms and concerns over the phone using clinical judgment and approved protocols. Determine the urgency of symptoms and provide appropriate advice, including referral to physician or emergency care.
Care Coordination:
Communicate with physicians, nurse practitioners, and other care team members regarding patient needs. Coordinate appointments, follow-ups, and referrals. Monitor and track pain management plans, urine drug screens, and prescription monitoring program compliance.
Documentation:
Accurately document all phone interactions and clinical decisions in the electronic medical record (EMR). Maintain up-to-date and compliant nursing documentation per clinic policy.
Qualifications:
Education:
Valid LPN license
Experience:
Experience in pain management, primary care, or a triage nursing role preferred.
Skills:
Excellent phone etiquette and communication skills. Ability to assess patient needs remotely. Strong organizational and documentation skills. Familiarity with electronic health records (EHR/EMR). Knowledge of chronic pain conditions, opioid safety, and triage protocols a plus.
