Your Key Responsibilities:

Conduct nursing assessments and monitor patient conditions Deliver medications, treatments, and skilled interventions Educate patients and families on safety and home care routines Assist with daily living needs while promoting independence Document care accurately and communicate updates to the RN and care team

Qualifications:

Active LPN license in Minnesota 1+ year of clinical or home care experience Strong communication, organization, and problem-solving skills Ability to work independently in the field Reliable transportation for patient visits
