Licensed Practical Nurse (LPN) - Nurse Case Manager / Staff Nurse CaringHands Assisted Living | Part-Time Job Summary CaringHands Assisted Living is seeking a compassionate, organized, and dependable Licensed Practical Nurse (LPN) to join our team in a part-time Nurse Case Manager / Staff Nurse role within our small, resident-centered assisted living community. This position is ideal for an LPN who values personalized care, clinical oversight, and building meaningful relationships with residents, families, and staff. The LPN will provide nursing oversight, medication management, care coordination, staff support, and clinical documentation while helping ensure compliance with Minnesota assisted living regulations. Responsibilities Conduct resident nursing assessments and monitor changes in condition Administer medications and nursing treatments per provider orders Review medication orders, reconciliations, and treatment plans Communicate with physicians, pharmacies, home health, hospice, and other providers Assist with admissions, service plan updates, and nursing documentation Supervise and delegate appropriate nursing-related tasks to caregiving staff Respond to resident health concerns and coordinate follow-up care Maintain accurate clinical documentation in the electronic health record Support infection control, medication management, and resident safety practices Assist with regulatory compliance and quality improvement activities Provide education and support to residents, families, and staff as needed Qualifications Current Minnesota Licensed Practical Nurse (LPN) license in good standing Experience in assisted living, long-term care, home care, or related healthcare setting preferred Strong clinical judgment, organization, and communication skills Ability to work independently in a small residential setting Experience with behavioral health, dementia care, or chronic disease management is a plus Knowledge of Minnesota assisted living regulations (144G) preferred Schedule Part-time | Flexible scheduling Join CaringHands and be part of a team dedicated to compassionate, person-centered care in a home-like environment.
