Administers medications, monitors resident health status, and ensures care is delivered in accordance with care plans and regulations. Supports a safe, efficient environment through strong clinical practices and teamwork. What You Will Do Administers medications and completes medication setup in accordance with physician orders and delegation guidelines Performs routine clinical assessments and reports changes in resident condition to RN or provider Supports development and implementation of care plans; follows and updates per RN direction Assists with new resident admissions, including data collection, documentation, and order coordination Supports discharge processes and transitions of care to ensure continuity and accuracy Completes narcotic counts and participates in medication audits per policy Documents clinical notes, incidents, and updates accurately in Residex Monitors care delivery and ensures services align with care plans and 144G compliance standards Participates in quality assurance activities and safety audits Communicates effectively with physicians, nurses, and interdisciplinary team members Builds and maintains positive relationships with residents and families Addresses resident and family questions and escalates concerns appropriately Participates in interdisciplinary team meetings and contributes relevant observations Supports training and guidance of care partners under RN supervision Reinforces best practices in infection control, safety, and resident care Maintains organized clinical supplies, medication carts, and documentation systems Promotes a positive team environment through communication and collaboration Provides nursing support coverage during both business hours and designated afterhours oncall rotations. Additional tasks as assigned.
