Job Summary We are seeking a dedicated and skilled Home Health Licensed Practical Nurse (LPN) Visit Nurse to deliver high-quality nursing care in patients' homes. The ideal candidate will possess strong clinical skills, a compassionate approach, and experience in various healthcare settings. As an LPN Visit Nurse, you will be responsible for assessing patient needs, administering treatments, and ensuring safe, effective care delivery while fostering positive relationships with patients and their families. Responsibilities 1. Demonstrates attendance and reliability for undisrupted care and services to patients 2. Able to adapt nursing principles and practices to home health care patients in a variety of home settings 3. Promotes, maintains and restores health using appropriate, preventative, rehabilitative and /or palliative measures by following the patients care plan. 4. Provide skilled nursing care interventions, according to the individual care plan as developed by agency RN or ordered by a licensed health care provider 5. Report changes in health status, responses to interventions, and any new orders received to RN Case Manager timely 6. Monitors client's vital signs, and health status/condition 7. Obtains and implements physician orders 8. Collaborates with family and other professionals to best coordinate care 9. Maintains and completes accurate documentation in a timely manner a. Point of care documentation will be completed with the expectation that at least 80% of the documentation is done in the patient's home b. Subsequent visits should be completed and synced in on the same day 10. Provides 5-7 skilled nurse visits per day as delegated by RN 11. Schedules/completes visits during regular business hours and per weekend/holiday rotation schedule 12. Participates in case conferences and/or IDT meetings if applicable 13. Attends staff meetings and educational programs as required 14. Follows State and Federal Regulations for Home Care and Hospice 15. Other duties as assigned Experience Candidates should have prior experience working as an LPN in diverse healthcare environments such as home care, long-term care facilities or skilled nursing facilities. Critical care experience—including ICU or emergency room settings—is highly valued. Familiarity with EMR/EHR systems like HCHB is preferred. Additional experience in hospice care, dementia or Alzheimer's care, medical documentation, triage procedures, and working with individuals with disabilities or developmental delays will enhance candidacy. Candidates must demonstrate excellent patient assessment skills, physiology knowledge, and the ability to manage acute pain situations effectively.
