Job Description

Our Cook Carefree Living community is excited to welcome a Licensed Practical Nurse (LPN) Care Coordinator to our dedicated assisted living team in Cook, MN. RESPONSIBILITIES
Provide hands-on nursing care including medication administration, treatments, and support with activities of daily living (ADLs) Work closely with the RN and leadership team to support individualized care plans Complete documentation accurately and in a timely manner Support Resident Assistants through mentorship, coaching, and positive leadership Manage medication orders, refills, and medication processes Perform supervisory visits and complete chart reviews Ensure compliance with company policies and Medicare Comprehensive License requirements Communicate effectively with residents, families, and team members Assist with ongoing quality assurance and regulatory compliance Other duties as assigned
QUALIFICATIONS
Current Minnesota LPN license required Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families Assisted Living experience highly preferred Familiarity with Alzheimer's and dementia care is a plus Confidence using electronic documentation systems Team-oriented mindset and desire to contribute to a supportive work culture Must pass a DHS background study and meet the physical requirements of the position
