LPN Visit Nurse Responsibilities:

Complete routine nursing assessments and vital-sign checks Carry out individualized care plans and provider-ordered treatments Administer medications and monitor patient progress Educate patients and families on disease management and safety Document care accurately and communicate changes promptly

Qualifications:

Active LPN license in Minnesota Minimum 1 year of home care or related clinical experience Strong clinical judgment and communication skills Ability to work independently Reliable transportation for home visits
